Changing Lives by Changing What’s Possible

 Continuum Pediatric Nursing Services evolved from a company that staffed nursing personnel in various health care settings, including hospitals and nursing homes.  After developing a special bond with a baby in a Neonatal Intensive Care Unit, some of Continuum’s nurses were asked to help transition this baby home to her family.  From this experience, Continuum began advocating for families, expanding the possibilities of home care for children who would have previously been limited to acute or long term care as their only options.  By caring for the first child in Northern Virginia to come home from the hospital on a ventilator, Continuum ultimately gained the expertise necessary to change the lives of hundreds of families throughout the Mid-Atlantic region.              

Today, over fifteen years later, Continuum is the preeminent provider of pediatric home care in the DC,  Baltimore, Pittsburgh and Norfolk metropolitan areas; having helped countless families maximize their health care benefits as well as maintain their home life with an ill child. In doing so, children who otherwise would have perhaps been institutionalized have thrived in the comfort of their own homes surrounded by the love and support of their families.   We are proud to be the most trusted name in home care for this special patient population.  Our patients’ diagnoses range from extreme prematurity to genetic illnesses to various medical anomalies.  Bronchopulmonary dysplasia (BPD), persistent pulmonary hypertension, apnea, impaired suck/swallow, short bowel syndrome, malabsorption, reactive airway disease, muscular dystrophy, and mitochondrial disease are among some of the specific issues Continuum’s patients face.  These medically fragile children are dependent upon high technology medical support including feeding tubes, tracheostomies, and mechanical ventilation.  To maintain the highest quality of care, Continuum leads the home care industry in offering training opportunities to ensure our clinical staff is always knowledgeable of progressive, best practices.

 Yet beyond our clinical expertise lies the real “secret” to our success.  We recognize that a successful transition to the home requires care and attention not only for the patient, but for the family and others involved in the life of the child as well.  For Continuum this means that we must be familiar with the needs of mom, dad, siblings, physicians, discharge planners, ancillary care providers and DME vendors in order to do our job well.  While our team members all have their areas of expertise, we each consider ourselves case managers for every child in our care.

 Management of clinical care is overseen by Directors and Assistant Directors of Nursing, all of whom have significant NICU, PICU and pediatric home care experience.  Patients’ nursing schedules are coordinated by Staffing Coordinators who average over five years tenure.  Our Recruiters work creatively to recruit and retain nurses during one of the nation’s worst and longest lasting nursing shortages.  Financial staff interact with payer sources to alleviate the burden on the family.  Ultimately, effective advocacy for appropriate, cost effective nursing support is paramount to all of Continuum’s staff.         

 Our qualifications and commitment to quality care are most prominently evidenced by our JCAHO accreditation.  But the endorsement we most prize comes to us through phone calls and letters from families and continued referrals from discharge planners, case managers and families themselves.  We will happily share these references with interested parties, and we look forward to extending this continuum of caring to your organization and the children who need us most.
