Why Case Managers Refer to Us

Over 25 Years Of Pediatric Nursing Excellence!

As the first agency to bring a child home on a ventilator in Northern Virginia, we’re proud of our long history of “Looking Out for the Little Ones” and supporting their families and caregivers.

Continuum Pediatric Nursing began with the insight that children’s medical outcomes would be dramatically improved with the benefit of skilled in-home nursing, surrounded by the support of family in a familiar environment. Today, we’re a leading pediatric home health agency, comprised of an integrated, highly trained team of medical professionals. We work closely with families and their medical providers to ensure every child reaches optimal health and wellness.

A Word From a Referral Partner

“As pediatric home ventilator specialists at Med Emporium, we recognize Continuum’s highly skilled nursing staff as a valuable partner for our Respiratory Care Practitioners, but most importantly for the patients and families together we serve. As collaborated healthcare providers, our joint commitment to excellence along with our high-level quality of care provides our patients a successful transition to home, an enriched quality of life, improved overall health and a reduction in emergency room visits and hospitalizations. The rewards of our work together are giving our patients and their families the greatest opportunity to not only stay home together, but to grow and thrive together.”

The “Secret” to our Success: An Integrated Team

Current Patient

A successful transition to the home requires care and attention not only for the patient – but for the family and others involved in the life of the child. Our team works seamlessly to deliver exceptional care and we all consider ourselves case managers; our job is to not only care for your child, but to understand the needs of Mom, Dad, siblings, physicians, discharge planners, ancillary care providers and DME vendors.

Management of clinical care is overseen by Directors and Assistant Directors of Nursing, all of whom have significant NICU, PICU, and/or pediatric home-care experience. Patients’ nursing schedules are coordinated by Staffing Coordinators who work tirelessly with our nurses to ensure the right fit, optimal coverage, and consistently low vacancy rates. And our financial staff interacts with payers to alleviate the burden on the family.

We’re proud of our Joint Commission accreditation, but the endorsement we value the most come from our community: phone calls and letters from families and continued referrals from discharge planners, case managers, and families themselves.
